Facilitating PCMH RecognitionAugust 21-22 | Washington, DCAbout the ProgramIn response to the recommendations of stakeholders and our experience with more than 1500 NCQA Recognized primary care practices, NCQA has renovated its patient-centered medical home recognition program. The 2011 standards are more patient-centered with a significant emphasis on the planning, managing and coordinating care for patients. They have a greater emphasis on continuous quality improvement in primary care practices, and align with Federal initiatives of Meaningful Use. Join us for a thorough review of the PCMH standards. We’ll practice scoring elements and examining documentation that supports an application towards becoming a recognized patient centered medical home with NCQA. The purpose of this educational program is to help participants: Examine the essential characteristics of a recognized patient-centered medical home  Identify the measurement and documentation criteria for each of NCQA’s updated requirements  Facilitate the NCQA survey and evaluation process  Seats are limited. Early registration is highly recommended. This seminar tends to sell out quickly.
