Facilitating PCMH RecognitionAugust 21-22 | Washington, DCAbout the ProgramIn response to the recommendations of stakeholders and our experience with more than 1500 NCQA Recognized primary care practices, NCQA has renovated its patient-centered medical home recognition program. The 2011 standards are more patient-centered with a significant emphasis on the planning, managing and coordinating care for patients. They have a greater emphasis on continuous quality improvement in primary care practices, and align with Federal initiatives of Meaningful Use. Join us for a thorough review of the PCMH standards. We’ll practice scoring elements and examining documentation that supports an application towards becoming a recognized patient centered medical home with NCQA. The purpose of this educational program is to help participants: Examine the essential characteristics of a recognized patient-centered medical home  Identify the measurement and documentation criteria for each of NCQA’s updated requirements  Facilitate the NCQA survey and evaluation process  Seats are limited.
