These standards are considered essential elements of a patient centered health care home and include 7 mandatory standards. The essential standards align to the Joint Principles of the Patient Centered Medical Home and address key requirements for all Meaningful Use requirements for electronic medical records, e-prescribing, and quality data submission.The URAC PCHCH Practice Achievement also offers additional standards that are optional, thereby allowing a plan sponsor, pilot program, or practice to require self-selected standards from the remaining toolkit practice standards. These additional standards are noted as optional in the achievement guide and can only be used to supplement, and not replace, the 28 key standards comprising the URAC PCHCH Practice Achievement program.URAC’s PCHCH Achievement includes 7 modules essential to primary health care:- Core Quality Care Management – Patient Empowerment and Engagement- Patient-Centered Operations Management - Patient Registry- Access and Communications – Enhancing Patient Access to Services- Testing and Referrals – Track and Follow-Up on Referral- Care Management and Coordination – Comprehensive Health Risk Assessment- Advanced Electronic Capabilities and Registry – Electronic Health Record (EHR)- Performance Reporting and Improvement – Performance Tracking and ReportingURAC’s PCHCH Practice Achievement offers practices two levels of recognition:- Achievement- Achievement with electronic health records (certified EHR also meets standards consistent with Meaningful Use)Who Should Attend:The workshop is recommended for health care organizations seeking to become auditors who will audit and verify practice capabilities to meet the URAC PCHCH Practice Achievement standards.
