Additional hours as necessaryTRAVEL:  5-10 days out-of-town travel annuallySUMMARY OF RESPONSIBILITIES:The PCMH Strategist will provide subject matter expertise related to care management and overall practice transformation to support the AAFP Patient-Centered Medical Home (PCMH) Initiative. The PCMH Strategist will develop tools and resources that can be used in a variety of channels (web content, articles, and editorials) necessary to effectively transform to a PCMH.The PCMH Strategist will participate in various internal committees and external groups focused on quality and PCMH including the Primary Care Collaborative (PCPCC) as well as providing support to the Commission on Quality and Practice.Other duties as assigned.REQUIREMENTS:Bachelor’s degree in a related field or equivalent work experience plus at least six years of directly related work experience. (Master’s preferred).  Knowledge of care management and care coordination experience preferred; primary care medical practice management and payment model redesign preferred; project management and ability to synthesize large amounts of information into meaningful tools or processes needed. Ability to write concise content that can be: 1) adapted for the web, publications, or for tool or resource development and 2) used as reports to key stakeholders. Ability to write effective grant proposals and create power point presentations; ability to apply primary care practice redesign; performance measurement and improvement; change management and improvement, and project management skills needed.
