Changing Relationships: How to Foster Effective Communication with Patients and Families

Communicating effectively with all patients and families is crucial in providing care consistent with the medical home model. Facilitated by the National Center for Medical Home Implementation, this webinar will identify effective visual, verbal, and written communication tools that can be used in practice for all patients and families, including those with limited English proficiency and health literacy. The webinar will provide pediatric medical home professionals with practical and concrete tools to communicate meaningfully and effectively with patients and families.

By the end of this webinar, participants will be able to:

  • Describe how language access can improve delivery of care within the pediatric medical home model.
  • Identify effective visual, verbal, and written communication tools and resources that can be used in practice for all patients, including those with limited English proficiency and health literacy. 
  • Discuss evidence-based and evidence-informed promising practices that leverage effective communication and language access to improve equitable provision of care.
