The Missouri Health Improvement Act of 2007 included a provision that the Department of Social Services should create health improvement plans for all participants in MO HealthNet to include coordinated care plans and be enrolled in a health care home. The Missouri Foundation for Health launched the Advancing Chronic Care through Excellence in Systems and Support (ACCESS) program in 2010, designed to improve the delivery of chronic illness care. ACCESS works with Federally Qualified Health Centers (FQHC), local Health Departments, hospitals, and academic health centers through a learning collaborative model to improve team care and care coordination.  In 2011, Missouri was awarded the first approved State Plan Amendment (SPA) to implement their Section 2703 Health Homes program. To accelerate the transformation to a Health Home model, the Missouri Foundation for Health and the Healthcare Foundation of Greater Kansas City funded an 18-month Learning Collaborative for behavioral health and primary care teams.

HB 879: Primary Care Transparency Act

This bill establishes the Primary Care Transparency Act, which requires all health insurance carriers to report to the Director of the Department of Insurance, Financial Institutions and Professional Registration (DIFP) their total health care medical expenditures and their total primary care medical expenditures for the year by March 1st each year. By March 31st each year, the DIFP must submit a report to the General Assembly. All individual patient data reported under these provisions is confidential and must be reported in the aggregate.

Greater Kansas City-Missouri Comprehensive Primary Care Plus

In 2016, CMS selected the Greater Kansas City region as one of the 14 markets that piloted their recent innovative efforts, Comprehensive Primary Care  Plus (CPC+). CPC+ is an advanced primary care medical home model that rewards value and quality through innovative payments that support comprehensive care. CPC+ is an initiative developed by CMS that transitions Medicare fee-for-service to value based payments in collaboration with Medicaid and commercial payers.

Blue KC medical home

In 2010, Blue KC started the Blue KC Medical Home program with some of the most innovative and forward thinking primary care providers in the region. The goal of the program was to deliver accessible, high-quality care while controlling costs. In a Blue KC Medical Home each patient has an ongoing relationship with a primary care physician (PCP). The PCP then leads a team of caregivers that take collective responsibility for overall patient care.

Legislation to Allow a New Model of Medical Care Goes Into Effect Today

In May, the General Assembly passed legislation that gives Missourians more freedom to choose the health care plan that best suits their individual needs. House Bill 769, filed by Rep. Keith Frederick, R-Rolla, and handled by Sen. Bob Onder, R-Lake St. Louis, sets up the Direct Primary Care model for health care delivery in the state.

$3 Billion in Medicaid Funding Held Up in Legislature

 If Missouri lawmakers fail to act in the next two days on a key piece of legislation, Missouri could lose nearly $3 billion for its Medicaid program.  

Lawmakers face a September deadline to pass an extension of the federal reimbursement allowance, or FRA. The FRA is a tax paid by all hospitals and clinics who provide for Medicaid patients. 

When Missouri’s hospitals pay FRA taxes, it generates roughly a $1 billion dollars each year for Missouri’s Medicaid program. The federal government then matches that with another $2 billion.

UnitedHealthcare currently operates 13 medical home programs in 10 states for the commercially insured population  These programs include more than 2,000 participating physicians and 300,000 members.
