Collaborative Ramps Up!

Dear Members and Friends:

As we conclude the second month of the new year and look forward to spring, I have noticed a quicker step and new energy here at headquarters for the Primary Care Collaborative. Our organization is healthy and growing, with a resolute focus on our mission and our relationships with our executive members.

I'm pleased to introduce you to our new Director of Operations (see feature on her below), and we recently welcomed several new Board members bringing fresh insights and valued perspectives. We continue to expand our executive membership and will work to ensure continued participation from both the public and private sector, staying true to our multi-stakeholder mission. I could not be more optimistic about the future of our organization and all that we will accomplish together this year. 


With the new political climate and possible repeal of the Affordable Care Act, now more than ever we need a multi-stakeholder coalition like the PCPCC to advocate for advanced primary care that is patient-centered, team-based, and high-value. 

Representing more than 70 public and private organizations, including payers, providers, non-profits and leading corporations dedicated to transforming health care, we are in a unique position to effect change. The power of our collective voice can make a difference, and together we will make a difference.

Today we host our monthly executive member briefing where PCPCC executive members will be among the first to hear the details of the new Primary Care Payment Models white paper, to be published by the Health Care Payment Learning and Action Network (HCP LAN) in the next few weeks.

Meet Our New Director of Operations
We are pleased to introduce you to our new Director of Operations, Ms. Krista M.C. Conley!

Conley is a senior executive and proven entrepreneur, dedicated to growing organizations with talent, systems and innovation. Having founded and exited two companies and brought a third enterprise to a national innovation award, Ms. Conley is a recognized innovation leader, with genuine relationship development skills and an ability to work with major stakeholders, both public and private sector. For the past six years, she has been a pioneer in digital health, focused on proving efficacy with evidence-based programming, as well as working closely with early adopters to exceed outcomes.

In addition to her entrepreneurial experience, Ms. Conley has worked in government, first with Senator Edward M. Kennedy on his Labor and Human Resources Committee, as well as with the Hong Kong Government, negotiating their first bilateral trade and investment deal with a U.S. State. Having lived in urban America, as well as rural America, she has a clear and keen understanding of the health care stresses and opportunities in America today. Ms. Conley lives in Arlington VA, where she manages and directs her two sons, James and Nicholas.

5 Steps to a Culture of Patient Engagement 

Person and family engagement is a critical strategy for improving health outcomes. The federal Transforming Clinical Practice Initiative aims, among other goals, to help medical practices adopt person and family engagement strategies at multiple levels.

Transforming practices engage patients and families in a variety of ways:
  • In advisory and governance activities;
  • In the policies for measuring individual engagement in health; and
  • In individual encounters such as medication management.
Engaged patients become activated patients, leading to improved patient health.
 
Earlier this month, the PCPCC hosted a webinar discussing steps toward changing practice culture to embrace patient and family engagement.

Sara Guastello, Planetree Director of Knowledge Management, and Susan Brown, a family caregiver, outlined five key steps that transform practice culture:
  1. Ask patients how they want to be involved and what makes them feel like partners in health: use patient and family interviews, focus groups, advisory councils, surveys and other tools for assessing patient and family perceptions of care.
  2. Empower the care team: recognize that the care team includes the patient and caregivers.  Have them participate in quality improvement and seek/recognize staff that take an inclusive approach to patient and family involvement.
  3. Engage family caregivers: encourage family presence and partnerships.  Provide tools to support families and patients, and offer ‘care for the caregiver.’
  4. Activate patients: use strategies to engage and activate patients, such as collaborative agenda and goal setting, shared decision-making, and teach back.
  5. Redesign practice: move towards multi-disciplinary team-based care, education on patient-centered care, office environmental design that encourages care and collaboration.

PCPCC Names New Board Chair,
Welcomes New Board Members
As one of the largest primary care advocacy organizations in the country, we take pride in representing more than 70 executive member organizations, and our work has always been led by an impressive Board of Directors

We recently announced the appointment of our new Board Chair, Jill Rubin Hummel, JD, President & General Manager, Anthem Blue Cross Blue Shield of Connecticut. Hummel has been a member of our Board of Directors for more than three years and most recently served as Chair Elect. She will preside over our 14-member Board.

The American Cancer Society works with primary care providers and health centers to provide training and technical assistance on evidence-based strategies to improve cancer prevention services, increase cancer screening rates, address access to care, and meet the needs of patients who are cancer survivors.
