Practices participating in the Transforming Clinical Practice Initiative (TCPI) are constantly aiming toward effective transformation through the implementation of meaningful, person-centered care. In support of this goal, TCPI recently released six Person and Family Engagement (PFE) performance measures that can be used to strengthen partnerships in ambulatory care. 

This webinar is part one of a two-part series that will provide attendees with an overview of TCPI's Person and Family Engagement (PFE) performance measuresIn collaboration with our partners at the Institute for Patient and Family-Centered Care (IPFCC), we will describe three PFE metrics:

•   Shared decision making
•   Patient activation

•   Health literacy
 

Hear success stories from a clinician and patient advisor at Cambridge Health Alliance highlighting how they successfully implemented these strategies. They will also share special resources and practice tools.
