Patient-centered strategies to drive health system transformation.

With the U.S. spending roughly 30 percent or $700 billion on unnecessary health care services, consensus continues to build around the patient-centered medical home and its critical role in achieving the objectives of the Triple Aim: better care, better health, and lower costs. More than 90 health plans, dozens of employers, 43 state Medicaid programs, numerous federal agencies, hundreds of safety net clinics, and thousands of small and large clinical practices nationwide have adopted this innovative model.

The PCPCC developed the following infographic to help fellow medical home supporters and advocates explain the benefits and strategies associated with delivering patient-centered primary care. The graphic is organized according to the five key features of the medical home model: Patient-centered, comprehensive, coordinated, accessible, and committed to quality and safety. It includes definitions for each of these features, sample strategies used by health professionals, employers, and payers, and their collective impact on the health system.
