NH Citizens Health Initiative Multi-Stakeholder Medical Home Pilot, which ended in 2011, was a collaboration among the Initiative's medical home workgroup, the Center for Medical Home Improvement, and the four private NH Health Plans: Harvard Pilgrim Health Care, CIGNA, Anthem, and MVP Healthcare, as well as NH Medicaid. In July 2010, the NH Citizen’s Health Initiative (The Initiative), which is staffed through the NH Institute for Health Policy and Practice (IHPP), launched a value-based health care initiative in the form of a statewide, five‐year Accountable Care Organization (ACO) pilot project. The Initiative is a multi‐stakeholder collaborative effort working for a health and health care system with better health, better care, and lower costs for all New Hampshire residents.

In 2011, Governor Lynch signed legislation (SB147) that employs a Medicaid Care Management (MCM) model for administering the New Hampshire Medicaid program, which began in December 2013. The managed care organizations (MCO) are responsible for coordinating all health care services for members through a network of providers. This involves enrolling members into a medical home, which is typically a primary care physician (PCP) who will be responsible for providing regular preventative treatment and ensuring the continuity of care. New Hampshire is extending Medicaid through a "private option" platform in which the state will use federal funding to purchase plans for low-income residents from the insurance Marketplace.

Anthem program rewards quality patient management

Anthem Blue Cross and Blue Shield’s Enhanced Personal Health Care (EPHC) program is being well received by local primary care physician (PCP) practices that are utilizing it.

The program empowers PCP practices to improve quality and outcomes and, subsequently, lower costs. Participating practitioners receive fee-for-service payments plus a monthly care coordination per member, per month (PMPM) payment.

CMS State Innovation Model Design Award - New Hampshire

The New Hampshire Department of Health and Human Services engaged a broad group of stakeholders to develop its State Health Care Innovation Plan for the Centers for Medicare and Medicaid Services (CMS). This model was developed through an on-going stakeholder engagement process that lasted over 10 months, and works to meet the Triple Aim of better health, better care, and reduced costs for New Hampshire residents. New Hampshire's State Innovation Plan focuses on supporting individuals who are either in need of or at-risk for needing Medicaid long term services and supports (LTSS).

Cigna Collaborative Accountable Care - Core Physicians

Cigna and Core Physicians have launched a collaborative accountable care  initiative to improve patient access to health care, enhance care coordination, and achieve the “triple aim” of improved health, affordability and patient experience. The program became effective January 1, 2014 and is Cigna's fourth accountable care initiative in New Hampshire. Collaborative accountable care is Cigna's approach to accomplishing the same population health goals as accountable care organizations, or ACOs.

Cigna and Derry Medical Center, an independent family medical practice, have launched a collaborative accountable care initiative to improve patient access to health care, enhance care coordination, and achieve the “triple aim” ofimproved health, affordability and patient experience. The program became effective January 1, 2013. 

Enhanced Personal Health Care Program - New Hampshire

The Enhanced Personal Health Care Program empowers primary care physicians (PCPs) to engage in those comprehensive primary care functions that move us toward a coordinated, evidence-based care model that has the greatest impact on achieving the triple aim of improved quality, patient experience and affordability.

The New Hampshire Citizens Health Initiative Multi-Stakeholder Medical Home Pilot represents a collaboration among the Initiative medical home workgroup, the Center for Medical Home Improvement and the four private New Hampshire Health Plans: Harvard Pilgrim Health Care, CIGNA, Anthem, and MVP Healthcare as well as NH Medicaid. The goal of the pilot is to value, prescribe and reward medical care that is tightly coordinated and of superior quality and efficiency.
