In September, 2010 a state law (NJ P.L. 2010, Chapter 74) was passed directing Medicaid to establish a three-year pilot demonstration for medical homes focusing on the frail elderly and those with chronic diseases. In response to the legislation, the Department of Human Services, Division of Medical Assistance and Health Services (DMAHS) entered into a Memorandum of Agreement (MOA) with the four Medicaid contracted managed care organizations (MCOs) in NJ to participate in a pilot to enhance or create infrastructure, within their networks, for medical home services. Three of these MCOs are also participating in the statewide Comprehensive Primary Care (CPC) initiative in New Jersey.

New Jersey has two publicly funded Behavioral Health Home programs and a privately funded primary care/behavioral health care project. New Jersey is currently working on the development of a Medicaid Health Homes program based on the experiences with these programs that will focus on individuals with behavioral health care needs.

Patient-centered care continues to deliver on promise of better quality care at a lower cost

Members receiving care from a doctor who participates in a patient-centered program are scoring higher on quality care metrics – at a cost that is nine percent lower -- than those members at traditional doctor practices, according to the 2014 results of Horizon Blue Cross Blue Shield of New Jersey's patient-centered programs.

New Jersey Health Homes

CMS approved New Jersey's Health Home state plan amendment in March of 2015. In a partnership with the Division of Medical Assistance and Health Services (Medicaid) and the Department of Children and Families, DMHAS launched health homes in select counties in July of 2014.  The BHH is a high intensity service targeting those with the most need.

New Jersey has two Health Homes, an SMI adult Health Home and a children's SED Health Home:

CarePlus NJ establishes first state-certified behavior health home agency

CarePlus NJ in Paramus announced Wednesday that it is the first state-certified agency in Bergen County to establish a “behavioral health home, a new model of integrated primary care for individuals with severe mental illnesses.”

CarePlus Chief Executive Joe Masciandaro said the CarePlus behavioral health home model will be more affordable and will enable individuals with severe mental illnesses to access primary health care services more easily.

'Medical Home' Approach coordinates services, care for kids with Autism

Among the many challenges faced by families of children with autism is how to coordinate behavioral treatment with other medical services.

That’s why the state Department of Health plans to fund studies of a new approach called “medical home,” in which an autistic child’s pediatrician works closely with behavioral and other specialists.

Primary-care practices and health insurers across the state are already adopting the “medical home” model as a way to better coordinate care.

UnitedHealthcare Patient-Centered Medical Home Program- New Jersey

UnitedHealthcare currently operates 13 medical home programs in 10 states for the commercially insured population  These programs include more than 2,000 participating physicians and 300,000 members. 

NJ P.L. 2011, Chapter 114

In August 2011, Governor Christie signed this bill into law requiring the  Department of Medcial Assistance and Health Services (DMAHS) to establish a three year Medicaid Accountable Care Organization (ACO) demonstration project.

State Innovation Model (SIM) Design Award - New Jersey

The state of New Jersey will recieve $3,000,000 to design a State Health Care Innovation Plan. The Model Design award recipients will engage a diverse group of stakeholders, including public and commercial payers, providers and consumers, to develop a State Health Care Innovation Plan. 

Sweeney Pilot Aims to Save Money, Improve Public-Employee Healthcare

Senate President Stephen M. Sweeney has unveiled a proposal for a pilot project that -- if successful -- could go a long way toward saving money on health benefits for state workers while keeping them healthy.

The pilot would start with as many as 60,000 of the 800,000 public workers in the state’s two major health insurance plans. Rather than a conventional fee-for-service model, the project is based on “patient-centered healthcare,” in which doctors are paid based on seeing a limited number of patients, with bonuses awarded for keeping those patients healthy.

Medicare initiative is paying off in terms of cost and care, particularly in New Jersey

New Jersey is among seven states nationwide in which Medicare is partnering with health insurance companies in a unique, four-year pilot program aimed at improving the delivery of primary medical care in America. And it's starting to show results: Medicare announced that the Comprehensive Primary Care initiative is having a positive impact on both the cost and the quality of care.

$148M could be saved if mental health gets better attention, study says

New Jersey taxpayers could save hundreds of millions of dollars a year and thousands of patients could have improved health and better quality of life if clinicians coordinated physical and mental health care, a new Rutgers University study says.

The report, produced by the Center for State Health Policy, found that more than a third of the $880 million in hospitalization costs in the 13 communities it studied were associated with behavioral health issues such as mental health disorders and substance use.
