Program Description:

The Clinical Health Coach training online program is designed to improve patient health outcomes by:  1) actively engaging patients in self-management support and inspiring greater patient accountability; 2) building capacity for effective use of population health processes in all patient-centered settings; and, 3) aligning evidence based care management strategies with patient-centered resources. This self-paced online program involves 27 hours of tutorials including:

  • Emphasis on evidence based patient engagement approaches through a health coaching approach
  • Exploration of a wide variety of health coaching skills and care management strategies that can be learned and effectively used by anyone on the healthcare team to achieve a patient-centric environment
  • Clinical assessments, tools and documents that support the guided care process and patient selected goals for behavior change
  • Population health process improvement to engage your entire panel of patients
  • Effective healthcare communications to improve health literacy and inspire accountability
  • Certificate of Competency in Clinical Health Coaching upon successful completion of coaching performance assessment

The program builds knowledge and skills for transforming conversations between healthcare professionals and patients for better self-management and transforming care processes by deploying population health strategies.
